Incident Investigation
Introduction
The motivation for investigating incidents is based on the concept of continual improvement by identifying risks and taking appropriate action to prevent incidents. In the event that an incident has occurred, the identification of the root cause and subsequent development and implementation of actions to prevent recurrence is paramount. One of the most common problems of SHE management lie in poor incident investigation leading to recurrent or additional incidents that could have been prevented or where losses could have been restricted.
Accreditation:
Application will be to HWSETA to have this course accredited against Unit standard 120335: NQF level 3, Credits: 5
Unit standard title: Conduct an investigation into workplace incidents
This is a fundamental unit standard.
Course Aim:
To provide a suitable learning opportunity to enable SHE professionals, supervisors and managers to effectively investigate Safety, occupational Health and Environment incidents.
Course Outcomes:
Learners credited with this unit standard will be able to
- Develop a causal factor chart.
- Analyse the causal factors using a root cause map.
- Demonstrate the ability to conduct investigations into workplace incidents.
- Accurately document investigation findings.
- Create an appropriate management report on the fact arising during the investigation.
- Develop an action plan to prevent recurrence or similar incidents.
Course Design
This is a full 2 day course covering several modules and includes a practical session. To obtain the credits and a certificate of competence there is also post course work that needs to be completed within 3 weeks of the contact session.
Assessment:
Assessment is done by Self, Peer and Facilitator evaluation throughout the course.
